CVE-2026-44628 is a remotely exploitable type confusion vulnerability in the OFFIS DCMTK Toolkit worklist server component, wlmscpfs, affecting versions prior to 3.7.0. An unauthenticated client can negotiate Explicit VR and send a crafted DICOM C-FIND request in which a dictionary-defined sequence tag is encoded on the wire using a non-sequence VR. In the vulnerable code path, wlmscpfs uses the resulting object as though it were a DcmSequenceOfItems without verifying the actual runtime type, causing access of a resource using an incompatible type. Under the documented conditions, this wrong-type cast/use crashes the process. The issue is triggered during worklist query handling and results in denial of service rather than reported code execution or data disclosure.
