CVE-2026-16971 is a missing brute-force protection vulnerability in the IRIS web application, confirmed in version 2.4.26 and possibly affecting other versions. The flaw affects the MFA OTP validation workflow, where the application does not enforce effective rate limiting, progressive delays, temporary blocking, or account lockout on repeated invalid OTP submissions. Testing showed that the MFA verification endpoint accepted rapid repeated guesses across the full 6-digit OTP space without observable throttling, and that response behavior allowed differentiation between invalid and successful submissions. Because the OTP space is small and the application does not sufficiently restrict repeated verification attempts, an unauthenticated or partially authenticated attacker can systematically guess MFA codes until a valid value is found, undermining the intended protection of multi-factor authentication.
